Dr. Manisha Singh, Consultant Gynaecologist and Subspecialist inReproductive Medicine & Surgery, Fortis Hospitals says that onemay not be able to conceive due to many reasons such as malepartner?s impaired semen quality, ejaculatory dysfunction, anatomical defects etc. It may also be the female partner?s tubaldysfunction, minimal to moderate endometriosis or inability to haveintercourse etc.
The IUI procedure starts with the assessment of the follicles(cyst like areas which contain eggs in the ovaries) from the secondday of the menstrual cycle via ultrasound scan. Stimulation offollicular growth can be achieved by giving hormonal injections ortablets. Follicular scans and blood tests help monitor the growth ofthe follicles.
HCG injection (hormone) has to be given 12-24 hours before theprocedure as the final trigger to allow for egg release from thefollicle.
On the day of the procedure semen sample collected by thepatient?s husband or a frozen donor sperm sample is washed andprepared in Andrology laboratory. The prepared semen sample (0.4ml)is loaded in an IUI catheter and inseminated slowly over a minute toavoid back flow.
After the procedure the patient is asked to lie down for half anhour for the sperms to disperse through the uterine cavity and tubes.Progesterone is given for 2 weeks and continued for 12 weeks if thepatient conceives. The patient should refrain from smoking anddrinking alcohol during the treatment and ensure that she has ahealthy diet.
Mrs. Shanthi, a patient at Creation said ?When I first met Dr.Manisha Singh I had already undergone 2 IUI treatment cycles plusother treatments in various hospitals but it was all in vain. We haddecided to undergo IVF at Creation and we met Dr. Manisha Singh. After reviewing our case notes she made us realise that IVF was notnecessary yet and she would prefer us to have one more go at IUI. Sheexplained that IUI is almost like a natural cycle and less expensivethan IVF. We took a chance and underwent the IUI cycle at Creation. The technique followed by the doctor was really different compared toprevious IUI?s which was done in other hospitals. After 15 daysfrom IUI to our surprise it was positive.?
Another patient Mrs. RV said ?We were trying to conceive frompast two years but we were unsuccessful even after trying an IVF +ICSI. We had almost lost our hopes but as one last try we walked intoCREATION. I and my husband were extremely happy and positivelyhopeful from the moment we walked in. Dr.Manisha was a wonderful andpleasing personality to interact with. She kept enforcing "extremelypositive" thoughts and feelings all through the interaction. Iwas apprehensive when Dr. Manisha said you guys are healthy andnormal. So I went for a few cycles of IUI. I doubted if it was theright thing to do. We are extremely elated now that I am in thesecond trimester.
IUI is a cost effective treatment and takes less time as opposedto IVF. The success rate of IUI is between 12- 20%. But few of therisk factors involved in IUI are multiple pregnancies, ovarian hyperstimulation and stress.
